The following appendices contain important supplemental information, procedures and forms and are an integral part of the Entomology IIPP document.
Safety training should be tailored to match the employee's job. The first part of this appendix outlines the guidelines that were used by the Department of Entomology to develop a set of Safety Training forms [ Appendix II, Forms Va - Vf ] that are based upon Environmental Health & Safety's collection of SafetyNets. The SafetyNets are organized by category [ e.g., Safety Orientation, Ergonomics, Physical Hazards, etc. ] to assist with developing customized safety training packages. To determine which Safety Training Package you need, a Safety Training Table is provided that will help you match the type of work with the appropriate Safety Training Form to use in developing a customized training package.
This appendix contains Safety Forms I-VII. Use the appropriate Form I-IV and VI-VII to document safety inspections, safety meetings, new employee safety orientation, etc. Use the appropriate Form Va - Vf to set up a SafetyNet-based training program for your work place [ see Appendix I ].
This appendix contains building evacuation guidelines, plus floor plans for each of the buildings in which units of the Department of Entomology are housed. Use these floor plans to create an evacuation route specific to the work area in your building.
This appendix contains information pertaining to the use of the Entomology Shop, located at the Harry H. Laidlaw Honey Bee Research Facility.
This appendix provides information on creating a Chemical Hygiene Plan (for laboratories that use hazardous chemicals) and the requirements for having Material Safety Data Sheets readily available in your lab.
