Volunteer Leader Requirements

4-H Volunteer Leaders are appointed annually by the University of California Cooperative Extension County Director upon completion enrollment. The three steps to appointment are:
1) Complete and submit to your club leader your Volunteer Leader Application (available from your club leader or found at http://ucanr.org/sites/UC4-H/files/4702.pdf.) This includes the:
a) Application
b) Health History form
c) Waiver of Liability, and
d) Volunteer Self Disclosure form
2) All volunteer applicants must successfully clear a California Department of Justice background check (Livescan fingerprinting) initiated by University of California Cooperative Extension prior to their initial appointment. To complete Livescan fingerprinting, you can go to any provider listed on the document found here: Stanislaus County Live Scan Instructions and Providers. Be sure to use the form with our county code numbers provided here: Request for Live Scan Service (print 2 copies) Up to $10 will be reimbursed if you bring your receipt to the 4-H office.
3) All volunteer applicants must complete a Leader Orientation. To complete a Leader Orientation, watch the video below and complete the Orientation Quiz (link is below the video frame). Be sure to complete instructions on your score sheet when you complete your quiz. (Click on the white arrow to start the video)
